How to Register with the Surgery
If you live within our
Practice area, and
would like to register, please come to the surgery to complete the
registration process or download appropriate registration form/forms by clicking on the
links below. Everyone should complete form 1, adults should also
complete form 2 and you should complete form 3 for children aged 5
and under.
We also need to see 2 forms of identification. Something
with your photograph on, for
example your passport or driving licence, and something with your
address on, for example a bank statement or a household bill. Once you are registered you
can access all our services.
Our registered patients can also make appointments and order repeat prescriptions online. For more information please ask at reception or your GP for details or visit the Emis Online Services page.
